Social Services

The Social Service Department at Windsor is dedicated to enhancing the quality of life of our residents by meeting regularly with residents, family members and caregivers to identify and resolve specific care and psychosocial needs through the process of advocacy and referral.  The Social Services Department also provides support such as transportation, community resources, room changes, and Advance Directives.  During the admission, the Social Service Director will be available to assist in making a smooth transition for the new resident and family.  The Social Service Department is part of the Interdisciplinary Resident Care Team that also participates in the discharge planning process.  Arrangements needed for Home Health services following rehabilitation are made by Social Services to expedite a smooth transition to home.
